Make NaN bit patterns fully non-deterministic.
It turns out that requiring NaN bit patterns to be deterministic with
respect to opcode and operands within a given run of an application is
too strict, because live application processes may be migrated between
implementations.
This patch also elaborates on the exceptions to IEEE-754 conformance.
